'No Hwk', 'Impertinence', 'Talk after 2 warnings' and 'Further talk' are among the future Beatle's transgressions.
Lennon was a pupil at Quarry Bank from 1952 to 1957. He used the name as the inspiration for his first band, the Quarrymen.
The sheet was saved from a fire at the school in the 1970s.
A historic contract the Beatles signed with manager Brian Epstein sold for £365,000 ($553,000) at the same sale.
